If you are meeting him/her for the first time in a professional setting then a more formal address would be appropriate, until he/she instructs you not to do so, or you are comfortable and others are addressing him/her that way. Or after a few interactions you can ask which they prefer.

If you are meeting them outside of work than a first name would be acceptable, if that is how they are introduced to you.

Always address him as Sir or Mr. unless and until he tells you to call him Ron. Even then, I would use discretion if I chose to call him Ron. I would never, ever call him Ron in front of other "executive" types, either. It is just a matter of respect.

It depends on how he/she wants to be addressed. If it is your first introduction, you should use Mr./Ms. If they tell you to call them otherwise, be sure and remember that, and call them by that name every other time following.

It depends on your position. Since you're not sure, play it safe, refer to him as Mr. ? If he wants you to call him "Ron," then he will tell you.
