There is nothing really wrong with you, it probably means that you are an intelligent person.
Part of this is certainly caused by ADD, part of it may be that you have a hard time making decisions and taking initiative. You feel that you have to break everything down and cover every possible angle before you commit yourself.
Of course you have to realize that you are far from omniscient. Most of the time you are just shooting yourself in the foot, especially when you simply don't have enough information, or you just don't know. Over-analyzing things just makes you paranoid. It probably makes you *more* likely to make mistakes, not less, because you are constantly second guessing yourself.
It is OK to be ignorant. In many cases, one choice is as good as another, as long as you have the courage to stick by your decision.
Try telling yourself that you really have *no idea* what the answer is. Try thinking "intuitively" rather than "logically"; ask yourself "what makes the most sense", and then stick with that conclusion. Just sit back and passively observe, until your observations prove otherwise. Your intuition is more powerful than you realize. Try thinking positive, start listing the positive aspects of any situation, rather than breaking apart it's potential downsides.
Meditation also helps a lot. Find a private space for yourself, sit down and relax. Close your eyes, and try to empty your mind of everything but a single idea or thought. Once you have some practice silencing that "internal dialog", you may be more successful suppressing it when it becomes intrusive. "Loud internal dialog" is often caused by excessive stress or insecurity, basic anti-stress and relaxation techniques may be helpful as well.
Apart from that, seeing a counselor or psychiatrist doesn't hurt either. Anti-depressants or stimulant-type ADD drugs can help enormously, but shouldn't be relied upon. Half of dealing with disorders such as ADD is actual treatment, but the other half is simply care and self-discipline.
Good luck!

The voice in your brain is just your thoughts...your "internal dialogue." Everyone experiences this, you're perfectly normal.
Actually, you can, if you learn the technique of meditation. The skill is to concenrtate on one thing, such as a sound, a candle flame or your breath, so that you can banish all other thoughts from your mind. When they start up again, you simply refocus. Do this for 15 minutes alone in a quiet room. Shutting your brain up will help you refresh your mind, almost the same way sleep does. It's a fanastic discipline. Try it sometime!

Have you tried any meds for your ADD? Or finding some outlet for your racing thoughts such as journaling or a physical activity? Maybe there is something on your mind that just needs to be talked out. Sometimes that works wonders?

Stop anthropomorphizing the brain as a homunculus. It's an important organ and participant in the behavior field, nothing more. Find a good behavior analyst that specializes in ADD!

Better living through chemistry. Are you on medication. Sometimes, it takes a little help to stop being so apprehensive. Check with a doctor. I have a daughter, who is ADHD, and she couldn't stop talking. I was that she was always saying what she was thinking about. I put her on medication and her talking was less, Not stopped, but less.

If you have ADD, then you probably need to take medication to balance out the chemicals which cause it.
