US destroyers depth charged Soviet submarines near Cuba, one sub nearly fired off their nukes but stopped as one of the 1st officers disobeyed his captain's orders...true story.

Oct. 27, 1962 - a Soviet submarine officer named Vasili Alexandrovich Arkhipov reportedly refused to comply with the launch of a nuclear warhead while apparently under attack by a U.S. warship near Cuba. In order to initiate such an attack, Soviet naval procedures stated that the captain and two other officers must concur. The other officer on duty agreed to the launch, but Arkhipov convinced the captain to wait for instructions from Moscow before proceeding. The orders never came and the world was saved from nuclear obliteration.

I would say the only occasions were when Soviet forces were disguised as foreign belligerents even when this was known by American forces.This was a method that could avoid an escalating direct confrontation whilst still influencing local conflicts.

Soviet Pilots were known to have been flying Migs over N. Vietnam. Some were also there in Korea. Several soldiers reported seeing Soviet ground troops during Korea. I was in Korea and in Viet nam and picked up on these stories. I don't know if they are true.
