Depends on whether I'm with my husband or not.
If I'm not, then I just avoid itching it and move along; most of the time it'll stop on its own. If it's really bad, then I always act like I'm getting something off the bottom shelf, then bend down and look at the item and quickly itch while no one is looking.
I'll also resort to acting like I'm getting something out of my pocket and scratch through the pocket, but that only works with certain pairs of jeans/pants.
If my husband is with me, then I always just act like I'm giving him a hug and put one arm around him and quickly scratch with the other. If for some reason I can't do this, then I just go back to avoiding it and hoping the itch stops quickly.

Make a loud formal announcement that your is itchy and could somebody help you with it. Thats what I do. Same goes for wedgies.
