Take it off with a wrench - use an old washcloth where the wrench's teeth might mar the fixture - and soak it in white vinegar or any household acidic cleaner, then clean with an old used toothbrush and toothpicks if necessary to open all the holes.

CLR - Calcium Lime and Rust remover work well. Just spray and wipe. If it is stubborn, but some in a glass just large enough to accomodate the shower head and hold it there for maybe 5 minutes. Wipe it clean with a conventional cleaner to stop the acid action when you are done.

CLR works the best. That's what I use. Just keep the shower head submerged in a small container of CLR for a few hours. It will be like new again.
