Having taught EFL/ESL for many years, I always started out with classroom vocabulary ie. sit down, open your books, please read, come here, take out your pencils... because they have to understand what they have to do in school. At the same time, I started with the studens' information ie. name, address, phone #. After that we moved out into community information....The best way to think about it is to imagine a spiral coming outward with a small starting point in the middle (student) I always started from there and worked my way outward. student - classroom/school - personal information - community - country....they learn each new step by building on the previous one and expanding their knowledge base in ever expanding logical increments. Good place to start looking for materials on these is to go to ESLCAFE.COM, where you will find thousands of lesson ideas on every subject and in addition there are many EFL teacher forums where you can post questions and ask for further advice. Good Luck, it's a lot of hard work teaching EFL but so much fun too.

Well I think the first thing is always getting to know each other. It depends on the students' level, and in most schools you know that before you even go into the classroom, so you can do it in many ways. Basic: teach "what's your name" and so on. Intermediate: revise all introduction questions and maybe add something a little more difficult, like an opinion question. Then get them to find out about all the other people in their class, and introduce each other to you, so you'll remember their names too. It also helps you see if they've been correctly placed and where they are within the level. After that it's up to you, you can go with the book or a piece of reading or a placement test or a prepared lesson on just about anything.

A good start to ESL teaching is to elicit what the students' English speaking level is already. You can introduce yourself by name, tell them where you are from then ask them by words and gestures to introduce themselves to the class, or to split into pairs and find out about each other then introduce their partner to the class. (Demonstrate this with one of the learners). A good milling activity would be to produce a sheet with pictures and simple words eg pizza, cinema, chocolate, swimming etc, and get them to find someone in the class who likes....and find out their names. You should watch and listen to all interactions to see how well they can communicate with each other.
