I received disability insurance benefits last year during my maternity leave. So does that mean I need a 1099-G form for my taxes? If so, how do I get one?

I am not a tax expert and would encourage you to consult the experts at the IRS web pages for this question...at a glance I thought the 1099G pertains to taxable distribution of funds. You received a benefit that is paid for by your insurance premium. The leave triggers the benefit to commence according to the terms of your policy. The money you got is I believe, a non taxable benefit. Unless you received a 1099G for this compensation; which would demonstrate that an amount was reported to the IRS as income; it's been my experience that disability benefits are not taxable income.

You would be likely to receive a 1099-G only if the funds received are considered to be possibly taxed. Sometimes, disability payments are not necessarily taxed because many other amounts you receive as compensation for sickness or injury are not taxable. These include the following amounts:
Compensatory damages you receive for physical injury or physical sickness, whether paid in a lump sum or in periodic payments. See Court awards and damages under Other Income, later.

Benefits you receive under an accident or health insurance policy on which either you paid the premiums or your employer paid the premiums but you had to include them in your income.

Disability benefits you receive for loss of income or earning capacity as a result of injuries under a no-fault car insurance policy.

Compensation you receive for permanent loss or loss of use of a part or function of your body, or for your permanent disfigurement. This compensation must be based only on the injury and not on the period of your absence from work. These benefits are not taxable even if your employer pays for the accident and health plan that provides these benefits.

What is a 1099G?

Form 1099G shows payments made to you by a government entity. The payment may be for such
things as serving on a government commission, per diem employment payments, or a taxable state
refund. It is the obligation of the payor to provide this form.

If they didn't send you one already, that probably means that the disability benefits aren't taxable - some are, some aren't, so nobody can tell for sure from the info you give here. You could call the agency you got the benefits from and ask them, they should know.
