what is the chemistry concept behind the effects of lidocaine?

I'm a dentist.

Lidocaine, like other "caines", is a membrane stabilizer. Specifically, it blocks the voltage-gated sodium channels at the nodes of Ranvier that allow the influx of sodium when the nerve reaches the threshold voltage (negative 40 millivolts, if memory serves), thus preventing an action potential. No action potential means no nerve impulse.

Because lidocaine blocks these voltage-gated sodium channels, which are located elsewhere in the body where action potentials exist (such as cardiac muscle), it serves as an anti-arrhythmic.

That's not all, though. Inject lidocaine next to a motor nerve, and it will create temporary paralysis.

When given in too high a dose, lidocaine's toxic effects are predictable: convulsions/CNS excitation followed by CNS depression, bradycardia, and eventually cardiovascular collapse.

Lidocaine is an amide, and is metabolized in the liver. It's active at the nerve in its ionized form, however, it reaches its target better in its non-ionized form. Beyond that, I don't know a heck of a lot about the chemistry.

Lidocaine alters depolarization in neurons, by blocking the fast sodium (Na+) channels in the cell membrane. With sufficient blockade, the membrane will not depolarise and so not transmit an action potential, leading to its anesthetic effects.
