It is very rare, but is still practiced in some cultures. Islam allows it only under very specific circumstances, and most men are not able to meet those circumstances.
I used to know a woman who was a second wife, and she was not jealous, nor was her co-wife. The two wives helped and supported each other socially, and the husband was very careful to avoid public display of affection which might have brought about jealousy. If the wives get jealous, either they shouldn't have agreed to the marriage, or the husband is doing it wrong.

The Quran says, "you can marry 2 or 3 and up to 4 women if you can be equally just with each of them" (4:3). Since it is very difficult to be equally just with all wives, in practice, most of the Muslim men do not have more than one wife. Prophet Muhammad (P) himself from age 24 to 50 was married to only one woman, Khadija. In the western society, some men who have one wife have many extramarital affairs. Thus, a survey was published in "U.S.A. Today" (April 4, 1988 Section D) which asked 4,700 mistresses what they would like their status to be. They said that "they preferred being a second wife rather than the 'other woman' because they did not have the legal rights, nor did they have the financial equality of the legally married wives, and it appeared that they were being used by these men."

It's not practiced much anymore... if your really religious and do it for the right intentions such as helping the person than it's fine. If your seriously doing it for the right reason your wife may not get upset. You have to have your wife's permission to do it also. So it works both ways.

[4.3] And if you fear that you cannot act equitably towards orphans, then marry such women as seem good to you, two and three and four; but if you fear that you will not do justice (between them), then (marry) only one or what your right hands possess; this is more proper, that you may not deviate from the right course.
Not many people can handle more than one wife.. Regardless if they are Atheist, Christian, or Muslim.

No one really knows...women are not allowed to leave their homes or their faces be seen in public. Besides women in Muslim countries are treated like no-entities. So I guess a man can have as many none-entities as he can afford.
