Quite recently (in 2004) it has been allowed, in presence of particular reasons, the Italians citizens to apply for the use of the mother's maiden name in addition to the father's name. However itsn't common that somebody avails himself of this possibility and generally speaking almost the totality of Italians has just the father's name.

It's not common for Italians to have a middle name. Only recently you'll find the younger generation with three names, but those cases are few and far between and also it's not common to find an Italian with the mother maiden name because Italy is a close knitted family oriented society and in almost all cases children born takes their father last name.

Most Italians don't have a middle name. It's just the first name and the last name. It's not traditional to give a middle name in Italy.
