I own a trim carpentry company - right now, for "basic" trim (setting doors, casing doors, window sills, closets, and base boards, and installing cabinets) I charge $1.50 per square foot of living area of the home. Staircases, mantles, crown molding, wainscott, chair-rail, library panel, built-ins, tub surrounds, aggravation fees, and any other custom work is considered extra. Of course, the pricing will vary depending on your location - and remember - the one truism in the construction business... You really do get what you pay for. There will always be someone willing to do the work for less money, but trim work makes or breaks a home, so DO NOT go with the lowest bid on this - if you do, I promise you will be sorry.

It depends on the trim. Cheapo generic stuff is like a dollar a foot, plus installation and finishing. Really nice stuff is like $10-$50+ per foot.
