I think it means the call didn't go thru because ciruits or switches are at capacity or there is a problem. For example, in places with an ACD (e.g. in your office), there may only be 20 lines going out, but 200 phones. If more than 20 try to get out, you'll get a fast-busy signal.

the called number could have a hard short on the line, and the person you are calling needs to call in a trouble ticket with the phone company, or it could be that there are not enough circuits available to complete the call (more common with nextel then any other phone carrier)

it could be that th enumber is disconnected or more likely that the curcits are busy and you need to heng up and try your call again

a reorder tone, (sometimes called a fast busy signal), indicates that no transmission path to the called number is available.
