How about starting a college fund for him. Ask others to contribute to the fund, instead of buying gifts.

It depends on your relation to this boy. If it is a nephew I would suggest something that you can continue adding to in the future such as a library of books. My daughter LOVES books and I have started a library for her to grow with. If books don't suit his family you could start with some really good DVD's. I have a collection of movies from when I was a child (all very good kid's movies) and my daughter is starting to watch them all and our collection of quality children's movies is growing.

If it is for a friend's baby, you could buy him a star from the International Star Registry. It's not too expensive- my husband got one for me years ago and you get a plaque with the star that is named after you and an official certificate. Basically, it's laying claim to a star in the universe in his name and you get the coordiantes so you can go to an observatory and look at it.

If you can make anything (a blanket, embroidery, etc) then that will make it unique because it's not something you can just buy in a store. My daughter received a blanket with her name and birth date embroidered on it, a cross-stitching of a teddy bear with all her birth stats on it from my grandmother, and my dad gave us a wood cutout of one of my favorite pictures of her just this past Christmas.

i would say definitely anything that can become an heirloom or tradition for your family...toys come and go (trust me, mom of 3 yr old boy)..whether it something special you make or buy make it unique and an expression of your love and even your talents..ru a good photographer, painter, etc..have fun w/ the project

Well...I think that this would depend on your relationship to the child. My son's grandmother got him one of those name print things that you frame and hang on the wall for his first birthday. Of course, she got the sterotypical toy too, but the name thing was special and he still has it on his wall! He is almost six now.
