1) entiendo / Entiendo lo que estás diciendo (informal)
Entiendo lo que está diciendo. (formal)

2) ¿entiendes? (informal) ¿entiende? (formal)

I understand = (Yo) entiendo

I understand what you are saying = (Yo) entiendo lo que tú dices.

Do you understand? = ¿(Tú) entiendes? (most common) OR ¿Entiendes tú?

The words in parenthesis can be said without it in Spanish and it will still make sense.
