I think that if a person is busy, it isn't rude. For instance, I work with children, I am constantly doing something and when another teacher approaches me and says 'Thank You', I either nod or say, 'mmm, hmm'. I am not trying to be rude, I just have other things I am trying to focus on. I do not say that to my children because I am trying to teach them to say 'Thank You' and 'Your Welcome' in reply, but when it's with another adult and I am running around, I just smile. Hopefully they get the picture.
I think that it isn't rude to an extent. And that extent is where you put it. I don't take it to offense when someone smiles or says 'mmm, hmm' but there comes to a point where I feel like I have to repeat my 'thank you' in a louder, more, 'I- am- waiting- for- a -better- reply-thank you.'
What it comes down to is your opinion of when it is OK and when it is rude.

Won't call it exactly rude, but I'll say that the person did not come from a family that is used to saying "thank you", hence don't know how to react.

Yes I would say so. What bothers me about todays society is how so few people today practice good manners. If someone says thank you to me I at least try to say your welcome, or simply nod in recognition. No one like a jerk.

A better response would be "You're welcome" or even the more casual "No problem" or "Any time," but I wouldn't get hung up on it. Everyone slips, especially when they're busy or mentally preoccupied.

I personally don't take offense to it. I am sure that I have been guilty of doing it every so oftern without even knowing it.

no... many cultures do not have an answer to 'thank you' as we do in the area i am in... which would be 'you are welcome'
the cultures are crossing into one another more and more... and what may be natural for one person to say.. is foreign to another because of their cultural background
