Surgery and medical methods could work but I would set them for the extreme cases like morbid obesity and such. A doctor should decide if that is the way to go, a decision that should not be made just because I want to look better, vanity, fashion, or business.
Going through such procedures is only a sort term solution in many cases because the causes that took someone to be obese or over weighted are still there.
In most cases (setting aside the real medical emergencies), the best way to go is to change lifestyles:
Sensible eating
Sensible cardio and toning exercise
Time to let nature take care of things and make them definitive, (and not just a patch in a torn cloth).
To get back into shape will take much less time that what it took to be over weighted, if done properly. This is a fact, the most out of shape some is, the faster they will make progress. It is not the first lbs. that are hard to get off, but probably those last two! But, if that is eventually the problem (the last two lbs), you are in not much of a problem, is´t is so?
My advice: no "shortcuts", better to lose weight and gain in fitness and health, in better living habits and general appearance and self esteem knowing we are bettering our self as time goes by and not just getting older and feeble.

No. It's lazy, unethical, and unhealthy in my eyes. You obtain something by the handing over of cash. It requires no effort nor will-power. No lessons are learnt, no respect is gained.
If you make the lifestyle changes you obtain not only a healthy body but a sense of pride knowing what you can achieve. You have respect for yourself and from others, probably jealousy too from those who haven't the strength to do what seems the impossible. You also learn the lesson of taking responsibility for your body. Be proud. Shortcuts are for desktops.
