Well, practically speaking, there are only a few things the doctor is looking for on the eye examination:

Conjunctivitis: basically diffuse reddening of the whites of the eyes -- this indicates inflammation

Icterus: yellowing discoloration of the whites of the eyes -- this indicates liver dysfunction

Cataracts: protein accumulations that are seen as opacities in the lens of the eye

Papilledema: seen as a bulging optic disc, this is probably the most important one, as it indicates increased intracranial pressure, which may become rapidly fatal

Detached retina: also an emergent situation, as the retina may soon degenerate

Light reflexes: both direct and consensual constriction in response to light -- loss of this indicates nerve damage/dysfunction

You have plenty of good answers. Probably the two areas of greatest interest in the back of the eye are the optic discs, checked for papilledema as noted by another poster, and the blood vessels. There are five pairs of artery/vein groups emanating from the disc, and it's possible to get a good look at blood vessels here without the interference of skin which obscures the view elsewhere. People with high blood pressure often exhibit particular changes in the blood vessels that can be seen on fundoscopy, and diabetics tend to have microaneurysms and grow extra blood vessels out into the vitreous (and get them zapped with lasers, thank God, these days).
