Bardzo chciałabym (chciałbym) zobaczyć jak się uśmiechasz. - I would love to see you smile.
in the 1st sentence: chciałabym (if the speaker is female)
chciałbym (if the speaker is male)
Na co masz ochotę? - what would you like?
if the 2nd sentence refers to a present, I'd say:
'Co chciałbyś/chciałabyś dostać?';
again 'chciałabyś' if you're asking a female
'chciałbyś' if you're asking a male

Her answer is from poltran.com and is crap. I don't know Polish very well at all, but I know it well enough to tell that what she gave you was from a machine translator and is a piece of garbage.
