A therapist can help with this question and will ask you some fundamental questions like:

- How long have you been in this relationship?
- How much do lover this person?
- What are the issues?
- Why do you want to break up with them?

Breaking up with a loved one should be done over a long period of time. You need to discuss the reasons with your loved one and hopefully you have been talking as problems have arisen.

If this is a loved one, you have a responsibility to yourself and your partner to wrap things up in a healthy way. You may consider discussing this together or individually with a relationship therapist.

Be honest as to why you want to break up. Don't just simply break up and walk out, and leave that other person hanging.

Prepare yourself for a discussion, as break ups are not easy and if this other person has become real attached to you there will probably be lots of questions and conversation to follow.

Of course you should be honest and thorough and be sure to make everything clear so you can end on good terms. Also including a good "this is hard to say, but.." will help a lot.
