Any nutrition store such as GNC (overpriced IMO). www.bodybuilding.com is where I usually go.

You can purchase creatine at GNC or other health stores, it's sold over the counter so you don't need doctor's approval. From experience though, I'd be very careful with that stuff. Its like eating 8 steaks in a 1/2 tablespoon, if you have high blood pressure stay away from this stuff, as it also creates plenty insulin at a level that can shock your liver.

out of your own body, that's right, Creatine is found in our muscles, its also in beef. who knows whats in the powder form, i wouldn't trust it especially when its hasn't been approved by the U.S.R.D. A..
