Ok, well practically, you have to be accepted to a residency program before your surgical skills are even relevant.

So, let's assume you're in medical school -- orthopedic surgery is one of the most competetive fields to try and get into, so you have to have decent grades. The USMLE step 1 exam is very important, as most residency programs look at that a lot. But probably the most important thing is that you have to know someone who can pull strings for you -- I'm not even close to kidding. Fields like ortho and plastic surgery are so competitive that you basically need to have someone usher you in -- i.e. form a relationship with an orthopedic surgeon and surgery ward at the hospital you want to be trained at, and hope that they will vouch for you when the time comes to apply for residency.

Take shop classes..no seriously, as an orthopedic surgeon, you use tools, many similar to those people who work in construction use.
You should also take art classes because surgeons have to have a good eye for detail.
Of course, take all the science and math classes you can.
Try to find a orthopedic surgeon who will allow you to watch one of his surgeries or at least let you go on rounds with him.

To be good with tools is very important. I put it akin to a carpenter of medicine. You have to put back broken bits as close as possible and get them to stay there. I work in the OR with OS all day in a teaching hospital. I see some guys (residents) who were book worms and never saw a tool and they have a TOUGH time. I even had one attending physician tell a resident he needed to buy a drill and go home and practice on wood before he could come back to the OR, it was a joke but he really wasn't kidding.
