where can i report a former empolyer who has failed to provide a w-2 for for tax preperation? this employer has been contacted numerous times with no avail.

Call the IRS and file a missing W2 complaint. Usually it takes just one call from the IRS to sort that out.

You can contact the IRS about it, and you can use a substitute W-2 form. There may be something on their web site, but eventually you'll have to call them, so just use their 800 number.
