When you got your license and signed for it, you agreed to submit to a test of blood, breath or urine if suspected of driving under the influence, and were told refusal would be automatic suspension of your license. Since you refused anyway, automatic suspension.. When they suspended it, they told you what you would need to do at the end of the 545 days to get it back. Do those things.

No, you are not able to get a permit. No driving for you. Once 545 days pass and if your license is still valid, your fine. If it expired you may have to re-test to get it validated. I think getting a permit is the least of your worries. Suck it up and admit to yourself of your wrongdoing and the lives you could have taken.

You will need to use a state identification card for i.d. until you reapply for your driver license. If you still have time on your driver license, they will simply issue you a duplicate license and the fees would be minimal. If your license expires during the suspension time, you will have to take the written test over again and the fees would increase.
