For a single line you only need two wires. On the phone it would be the green and the red. (A second line is yellow and black.)
Try the color pairs on the jack, one by one.
Be aware, that on touch tone phones, you get a dial tone either way, but the touch tones only come with the wires hooked up one way.
So when you try the wires from the jack, dial tone will tell you which are the right ones. Once you hook them up, see whether the tone is heard when pushing a button. If not, reverse the wires (one side only)

You just need a single pair to make a phone jack work. The additional pairs are for multiple lines. Just pick one pair usually the blue and blue/white. The blue wire goes to the red, and the blue/white wire goes to the green. You have to use the pair thats hooked up to the phone company though. It doesn't have to be the blue and blue/white, it could be any pair. The solid color wire in the pair would go the red, and the striped wire goes to the green.
