Although it varies somewhat from state to state, generally some kind of ID to prove you're a given age (and then whatever the age is for that state). I'm not sure how being underage would work, as some shops may refuse to do work regardless of the presence of legal guardians/parents.
The place I go to here also has you fill out a consent form.
You may wish to check out the shop you're looking at and ask. That would be easiest. As the saying goes, "We reserve the right to refuse service".

If he gets a tatto just tell him to go with a guardian if he looks that young.

you have to have ID no ID no tattoo no matter how old you look I am 27 and have seven tattoos most places actually take your ID and make a photo copy of it and it goes on the form you fill out so unfortunatly your friend probably wont be getting a tattoo....Good luck with this one!!!!!

You're gonna need ID for a tattoo or piercing no matter how old you are. I've seen people who are noticably older than me get carded, its policy.

Unfortunately, you need photo ID. I'm 23 (and look it) and when I had my last one done a couple months back, they made a copy of my drivers licence, its to protect themselves from minors using fake IDs. He could use his passport, if he has one.
