Alitalia is the only one with nonstop service on that route. They are the national airline of Italy and have improved quite a bit over the last few years in service. I don't believe American Airlines flies into Milan, unfortunately - only Rome.

You'd have to go through a hub airport (either a European airline through their hub in Europe, i.e. British Airways through London or Air France through Paris) or a US airline through their hub on the East Coast (for example, Continental through Newark or Delta through Atlanta). Going nonstop is best of course, so unless it's considerably cheaper to make a connection I'd go with Alitalia. They're not the best of the best in terms of service but they're good, and not having to make a connection counts for a lot.

i might settle on on Orlando. South sea coast is cool and is the playground of the prosperous and well-liked, yet except you're a factor of that team you would be very disenchanted. Orlando has a great nightlife, a lot extra fee-effective lodging and is often some levels cooler and is a little extra risk-free against storms than Miami is!
