Frequency of fuse replacement required to avoid any failure of equipment

Fuses aren't replaced as a part of planned maintenance. Essentially they last almost forever.

Fuses are used in circuits for protection from over loads,transients etc. Always use proper fuse ratings depending upon the application.
Fuses may not be replaced frequently if it is intact.Replace only when it is blown up.Replacement of good fuses will not improve the system parameters.
Please think about the fuses only when
(1) loading to the system is heavy , and
(2) faulty conditions

You would have to look up the cumulative failure rate for the fuse under normal loads, account for transients that exceed that and decide what number you can live with. You will never achieve zero failures even if you use redundancy.
