From the National Cancer Institute, the 7 warning signs of cancer are--
C- Change in bowel or bladder habits
A- A sore that does not heal
U- Unusual bleeding or discharge
T- Thickening or lump in breast or elsewhere
I- Indigestion or Dysphagia (difficult or painful swallowing)
O- Obvious change in wart or mole
N- Nagging cough or hoarseness

Do you mean scratch or itch? If itching is the problem, it is very common and I don't know the cause. Funny thing is-I had a mastectomy 16 years ago, due to breast cancer, and the place where the nipple would be if I still had a breast, itches once in a while.

You don't give many details, but I'm wondering if you have leakage from your nipple, which may be drying and causing your nipple to become rough and to scratch.
