Try these 3 things. I hope should work.
1. Use a good menthol tooth paste.
2. Use Dental Floss every day after you clean your teeth.
3. Always keep some Cloves in your pocket and when ever you get a chance chew them. Don't just eat them. The liquids produced from Cloves got a wonderful ingredient which not kills bad breath also the bacteria that creates bad breath.

last few post have good suggestions. You seem sure that its your stomach, so I guess youve already done this test. If you close your mouth and exhale strongly through your nose into your cupped hands, do you smell the smell in the vapours?
What does the smell, smell of( you can edit your first post to let us know)
You could try deodourising pills like Nullo, chlorophyll or activated charcoal to neutralise the smell
If the smell is coming from your stomach or gut, have you found certain foods increase the strength of teh smell. Could be a reaction your food, or bacteria in your gut
Finally, you could search for halitosis forums on the net, where fellow sufferers may be able to better advise you

Several of the responders have mentioned good oral hygene: brush your teeth, floss, and brush your tongue (top and bottom) and inside of your cheeks.! Bacteria in the mouth can cause bad breath. Check this site: http://www.ada.org/public/topics/bad_breath.asp
Do you have post nasal drip? That is, always have mucus or plegm in your throat that you have to spit out? this can be caused by sinus infections; and if it has lasted 18 years I guess you could call it chronic sinusitis. If it is sinus related, then clearing up the sinusitis will greatly improve your breath.
Post nasal drip can also be caused by allergies. An OTC drug like benedryl and pseudophed can help with that.
what is your blood type? If you are type B or AB, then you should avoid milk/dairy products as they increase mucus production. reduce the mucus, improve the breath. Based on the "Eat Right 4 Your Type" diet by Dr. D'Adamo, HP
First step: keep your mouth clean.
Second step: see physician to rule out infection or other abnormality.

Most of the problems of the stomach and mouth arise as a result of less consumption of water. Drink atleast 4 litres of water daily and see the results for yourself. In just two days to two weeks, your bad breath should vanish.

It seems like you would have to address the stomach problem via doctor visit, then that would improve the breath if he could prescribe something or fix your stomach somehow.

Perhaps if it's stomach related you could try an OTC like Prilosec or other GI med that reduces stomach acid contact.
