If the phone number you have starts with 0 (like 0451-466xxxx), leave out the zero. It's only used for long-distance calls within Germany.

Purchase a phone card is indeed the best way. Most long distance plans in America do NOT dial overseas. You cannot dial international on most cell phones. The bill from the phone company might shock you when you tell the Operator to make the call for you, like $7.50/minute. I had to pay for overseas dialing to have it added to my American long-distance. The other answers tell you correctly on the country code to dial.

Dial 9 then +49 then the code for where you want to call, omitting the first zero.
