Prior to being spayed she was fully house broken and hadn't had an accident in 2.5 months...

Yea, that is normal. In a few days she should be back to normal after she heals a little more.

Pop her to the vet to make sure that there isn't an infection which may be causing her to have accidents. If she is given the all clear then I'd wait a few more weeks for the surgery to have healed a bit more, if the accidents continue speak to your vet again.

Spaying can sometimes damage the muscles around the vagina causing incontinance. If you don't see an improvement in a week or two, check with your vet. There's a pill available called proin that is giving once every 2-3 days that will help.

This is quite normal for dogs to have accidents after being fixed. This mostly happens because of stress on the dog's body. You will have to re-train her to go outside.

Yes. Sometimes surgery weakens the muscles a bit. Return to whatever you did initially to housetrain her and she should be back on track soon.
