Ask the company. There is virtually no table that allows such a small deduction. Sounds like an error.

That depends upon your gross income and the number of withholding exemptions you are claiming. If your income is modest or you have a large number of withholding exemptions that would be entirely appropriate. If you're a single taxpayer, the first $8,450 of wages isn't taxed at all.

Probably you indicated "exempt from withholding" on your w-4. I have seen HR dept.s get it wrong, wrong and when the taxpayer says, I don't want to claim any exemptions, the person "helping" them puts exempt.
www.irs.gov, individual, withholding calculator will give you the correct withholding and you can even print out a copy to give your employer.
