I wouldn't say just go and ask her ,rather I would say make a situation to ask her out .
One good example : - A new movie released in this weekand , tell her how much you want to go for the movie ,just because it's about love and intensity on it or more about action something that you like to watch ...
Then continue by telling that it would be really boring that if you have to go alone bcoz make out a reason that all your friends are engaged with there girl friend or boy friends :)...
Finally showing out your reluctance just bcoz you are alone sweetly ask her " Will you come out with me for the movie , that will make me have a company "
About the date :- Make it more remembered one so that she will come out with you always ;)..like going to restaurant and ordering what she likes most .. etc..

Take the trouble to findout what your girl likes. My girl's weakness was the premier show of her favourite hero, I got the tickets, and the girl could not say 'no', in fact she was so impressed and happy that I got the date and I got her too.
