It sounds like you have very severe dry skin, and in the winter (dry) months it can become much worse.
First you need to invest in some good creams. There are lots out there, and you may need to try many before you find the one that works for you. My son has horribly dry skin (we call him gator-boy), so we have used: Nieva Visage (comes in little tins, very thick and waterproof); Aveeno Oatmeal formula (stops the itching, but wasn't moist enough); every type of Jergens (too thin, washes off easily, not enough moisture); and Vaseline (dermatologist recommended, believe it or not).
Now we use a combination of the following: We shower him with moisturizing body wash, and use Olay Shower Lotion. After patting him dry, we rub him down with the Aveeno Oatmeal moisturizer, let it soak in, then coat him with a VERY thin layer of Vaseline and rub it in. This helps trap the moisture in the skin, and has kept him from flaking/cracking all this winter.
If it doesn't get better soon, I suggest a dermatologist visit, as you can get really bad staph infections in your hands if they are cracked and raw chronically.

You may have a more serious skin condition than one that simply requires moisturizer. I would suggest using a heavy product such as lucas pawpaw ointment (available online or through medical distributers) I would also reccommend that you carefully read the instructions of any cleaning or personal products such as soaps, and try to stick to more natural products while you are suffering from this.
If all of this fails, see your doctor as severe skin conditions can often require prescription medications.

It sounds like severely dry skin....which means that it's time to go to the drugstore and get some high quality skin cream....I like glaxal base....it's non-perfumed so that it doesn't irritate your hands.

I would use some really good lotion and then use some surgical glove on hands and let them soak in the lotion. I hope that will help.

Always use hand lotion.
I like mine with Coco butter.
When you go outside this winter wear gloves.
