First, SpywareBlaster is not an real time anti-spyware program. It is an immunizing program. What it does is add a huge list of known, bad websites to to IE and Firefox, so these sites can't use Active X (IE) or use cookies. It doesn't run in memory like classic antispyware programs (Spybot, SpySweeper, etc.) and watch over your entire system. It's a great program and everyone should use it, but it's not real time anti-spyware.
AVG is good, but it's more of an anti-trojan. It's actually Ewido's anti-trojan engine that was bought and rebranded by Grisoft. They've added some anti-spyware functionality to it, but it's not one of the best. Also you said you wanted real time protection. AVG is only real time during the trial period. Once that runs out (30 days) the real time ends. Check it out for yourself:

If you want effective real time anti-spyware, your best bet is to get a paid program. They're a lot hardier then the freebies. Ones that I would suggest are:

The resource bloat of version 1.5.8.2 is gone. This is a new product from the ground up as developed by Eric Howes, a leading expert in computer security. I was a beta tester for this program, and I found it to run light and fast. I passed all the tests in the Spycar anti-spyware test suite with it.

The other is a newer program that is getting great reviews, Superantispyware

If you check out the security forums (Wilder's, Castle Cops) you'll see a lot of great reviews for this program from some serious security geeks.
Another one that's getting high praise, one I also tested, is Prevx1. Again superior reviews in the security forums. It relies on a central database, that it's always connected to, to check any process/program for malware. It compares signatures with every other user of the program. Sort of like "It takes the village to raise the child" idea. Resource usage is a bit higher then the others, but not bad (about 30MB on my machine).
Webroot's Spysweeper is a popular program but, I and a lot of others have stopped using it because of high resource usage and bugs. It's just not the same lean, effective program that it used to be (version 4.5 was the best).
Having said that, if you really want a free program, I would check out Spyware Terminator.

a site known to be very critical of software. Honestly, it's not as good as the paid for programs listed above, but if you combine it with the free version of Superantispyware (scanner only), you should be fine.
You might want to consider a HIPS (Host Intrusion Prevention System) that will notify you of any unauthorized changes to your system (registry, start up folder, system files, etc). One of the best and most established free ones is Winpatrol, found here:
