Or is there any age restriction on when a child has to get a passport?

Children require a passport to enter into the US.

HOWEVER, some countries allow you to add your children onto your passport instead of getting them their own until they are an adult. This type of passport will work also, allowing the children to enter into the US.

Absolutely, unless you are U.S. citizens returning from a U.S. Territory, all travelers are required to have passports. You'll have to contact the passport office in your home country to obtain details about acquiring a passport for your child. My daughter was one month old when we obtained her passport, as we were traveling abroad, her passport is valid for five years, as opposed to ten years for an adult. As far as I know, there are no age restrictions.

Yes sometimes you can get away with identification of some sort but I wouldn't risk it.
