They're in yourself. You make your own heaven and hell. When your mind is so diseased with bad stuff and you can't feel any joy, that's hell. Nothing any outside force can do to you can be worse than that. But when you've got love and you know where you want to be and you're learning - that's heaven. It only means something if it happens here on earth and you still have a chance to break free of the bad stuff.

Heaven is in the spirit realm. It is invisible to the human eye. The location is irrelevant.
"Hell" is the common grave of mankind. It, essentially, is death itself, not a place of burning torment. So, technically, hell is anywhere you see a graveyard.

I answered this one before but you obviously didn't see it. Heaven and hell are within us all. We create our own heaven and our own personal hell by our outlook on life. It would be so nice to think that we would be heading to heaven amongst the cushy clouds after we have done our good deeds, but tell me this when you feel peace within, how heavenly do you feel? And when you are in turmoil, how close to hell are you.

To lead humanity into a new human experience in a new World? after all it has become cheap or less expensive, to build a new earth for Jesus and for his followers, that push Satan out way from this area of Space and time. So this real state will become Satan's own world, the underworld if you please, or Hell.
