In business, it's unprofessional to ask someone to hold the line while you pick up another call. Not only that, but then you'll be faced with the decision of which call to terminate.
I think it's best to ignore the incoming call while on the phone with a client or business associate. Just let your voice mail pick it up, and return the call immediately after you hang up with the first person. In my opinion, that is the only professional way to handle this situation.

at the first sign of a pause, ask if he/she will hold as you need to answer another call, and advise the 2nd caller you have a call on hold and you'll call the 2nd caller back as soon as possible.
politely interrupt..i hate to interrupt, but i have another call. will you hold on? advise 2nd caller you have a caller on hold and you'll return the 2nd caller's call as soon as possible.
in any scenario, its always best to ask the first caller if he/she is willing to be placed on hold. never assume they'll be in agreement to being placed on hold. never leave your caller on hold too long, no more than a minute or two without getting back to them, even if it is to terminate the call. some callers have gotten all of the answers they need from you and just want to chit chat.
