I would advise removing the toilet, I've been installing ceramic tile for years and it just doesn't look natural butting the tile against the toilet. The only thing you'll need is a new wax ring and maybe a new toilet flange. The toilet is quite easily removed and the floor will look a lot better with the toilet resting on top of the tile. Just in case you might have to remove the toilet later you won't be breaking tiles at the same time. When re-installing just make sure the toilet is seated properly and also put a bead of silicon sealant around the base of it.

You don't have to, but why wouldn't you? Think how labor intensive that would be to cut tile in that shape all around the toilet. If you remove the toilet, the tiles won't have to be cut so precisely. Just a small hole of 4-5" is all you need to cut out, and it can be a rough cut because it will be covered by the toilet base when reinstalled.

Yes you do.

If not the toilet will sit lower than the tiling. You need to put a half inch or so of the tiling under the toilet to make it level.

Yes you do. Be sure to read how to do it and buy a new wax ring for reinstalling after the tile is done.

This tricky since it takes two days. The toilet has to be out until the grout is done and dry.
