With a few very specific exceptions, no one can insure your life without your approval. Also, the initial beneficiary must have an insurable interest in you. In other words, the applicant must demonstrate that the beneficiary would suffer a monetary loss upon the death of the insured. The beneficiary can be changed, but only after policy issue. You have nothing to worry about.

Only a parent of a minor child can take-out life insurance without the insurer knowing about it. Your signature is required in order to obtain life insurance.

Life insurance with cash value don't pay out cash value when you die! Sure you get face amount that is reduced by any loans and missed premiums, but you lose all the cash value! They say its a good way to build savings! How is that so if you lose it all and it doesn't go to anyone when you die? People say you can borrow it. Why do I want to borrow my own money that I paid for? Cash value = scams!

Well, you find out when they bring you the APPLICATION to sign, or when the nurse comes to your home or office to DRAW YOUR BLOOD, and when the insurance company asks you to HAVE YOUR DOCTOR RELEASE YOUR MEDICAL RECORDS TO THEM.
Someone can't put a policy on an adult without their knowledge, permission, and cooperation.

If you are an adult this cannot be done legally without your signature. So if someone has done this without your consent, you probably won't be able to find out, but I'd sure check out the person who you suspect of doing this.
