You are never required to send a gift, whether you attend or not.
You should contact the sender of the invitation and inform them in polite terms that you regret you will not be able to attend. No reason needs to be given.
If you are close to the person, you may want to send a gift; if so, that is entirely at your discretion.

Declining politely (a reason for declining is nice, but not required) and a simple card of congratulations or a "thinking of you" type card would be acceptable. If the person is especially close to you, a small gift just to make it known you appreciate the invite/your thinking of them/congratulations etc. is nice, but, again, not required.

The way I understand it--you are never required to give or send a gift. A gift is something you give because you want to.

You are not required to send a gift. It's a nice gesture if you do but you don't need to feel obligated. Many people send a cheque with their reply but mostly when it's family.
Bottom line, send something if you're close to them or feel like you'd like to, don't if you can't or don't want to.
