irrational fears, constant worry, crying, feeling lonely all the time for no reason, scared to go places alone, feeling guilty all the time, praying to get through work, feeling scared to go or leave or to even wake up and face another day knowing it will be the same, feeling discomfort and sadness when around others- especially when leaving their home, being your own worst enemy, not taking care of needed responsibilities at home, feeling like everyone else is normal, feeling numb, scared to even move in fear of a mood change, taking things to an extreme obsession for awhile such as doing one thing over and over until burnout occurs, getting a good feeling when helping others but can not help yourself, scared to learn or try new things and seeing that doing small things such as bathing is a major accomplishment-- and nothing seems to work like therapy and medication.
What would this person's problem be? I think it has been this way since childhood and got worse as an adult.

This person is suffering from severe depression. It also sounds to me like they have been this way so long that they are afraid of getting better. If they have been on medication and in therapy and have been honest with their therapist, they certainly should have seen some improvement. Each of the symptoms you stated are easily treatable IF the patient wants to get better. There are some patients who won't show improvement for fear of not receiving the attention they are used to receiving. This is a self esteem issue and I'm surprised that some psychologist hasn't figured this out yet.

Sounds like you may have ocd (OBSESSIVE COMPULSIVE DISORDER) google YBOCS, complete the survey, make an appointment with a doctor ASAP. Cognitive behaviour therapy could help. As far as the medication is concerned, it's alot of trial and error till a doctor finds the proper med for you as well as the proper dosage.
