First, look at what you are doing now, and decide if higher grades are important enough to sacrifice for. Mania about grades can get to be an obsession as harmful as any other in your life. And, sometimes you really can't control what grades you get, because of circumstances----the tension and anxiety surrounding the "wait" for the semester grades can take a toll on your health and emotions.
Look at the long view, and try to have a well-rounded school life, but don't harm yourself or others emotionally in a battle for grades. If you get your priorities straight, and pace yourself, it will also be a lot easier emotionally and physically to keep up with school and stay enthusiastic about studying.

GUIDELINES FOR STRAIGHT A s...
~take clear and understandable note. (and review them)
~do not daydream in class!
~try sitting in the front if you tend to be distracted or bored easily.
~DO NOT PROCRASTIATE! (this is important; managing time wisely is something everyone needs!)
~actually do the practice problems at the end of each section, they are similar to questions that show up on tests anyway. (but do not do every single question... that takes too much time, just one or two for every different skill learned, just to make sure you understand)
~ASK if you do not understand. (questions are meant to be asked, don't just let it go)
~get a planner to write down upcoming tests, homework that are going to be due, and time tables that you can follow, like: Tuesday... study for math test from 4 to 4:20.
~you don't have to study all day, just as long as it takes for you to understand and be familiar with the material.
~REVIEW the lesson or read the section the afternoon it was taught, not days afterwards, this way you wouldn't forget what the teacher said. (notes would be important if you do forget)
~take some time to have fun too, but not all the time.
~sleep, exercise and be healthy, a strong body carries a strong mind.
~make friends or talk to classmates that are smart in subjects you don't understand and ask them to help you. (you should really be their friends and help them in return in something else)
~ask your councilor to change a teacher if you do not understand his/her lessons and do not do well with his/her teacher methods.
~AP/HONORS: if you really suffer with advance classes and almost fail in them even though you try very hard, you should take normal classes. There is no point in taking them if you don't learn anything. An A in a normal class looks better than a C in an advanced class on a college application anyway.
I hope this helps, it works for me, so I hope you would find this information useful too! =)

use flash cards to help you study,
try not to study with others b/c it breaks the concentration and you'll be talking about other things. get a good night sleep before a test,
Try to reright your notes and highlight with different collored highlighters so the important stuff sticks out so on a test you remember more of the highlighted stuff b/c the colors stand out. There is such a thing as over studying if you find yourself reading the same thing over and over again and you still don't know what your doing, put it down and come back to it an hour later.

No one can really force you to study, so if that's not your thing you could try just straight out extra help like staying after school.
If you find that your doing good on tests you must remember alot of teachers will grade participation (so don't goof off in class), and homework is always a must especially for teachers who don't give much, so don't give away an easy grade booster.
And extra credit can come from anywhere so always be asking teachers for it, if your willing to do it.

Make sure you study every day. Even if you dont have homework in a subject take your books home and review what you learned in class that day.. daily review works much better than cramming at the last minute.

You obviously have other things on your mind. So, yes, you need to excersize to clear your head. And when you study, turn off tv, radio, etc.. And start back to basics, including writing something over, re-read the chapter, re-do the problems on a clean sheet. Gear your mind to study time and do it!!
