Our prayers rising to God.

"And another angel came and stood at the altar with a golden censer; and he was given much incense to mingle with the prayers of all the saints upon the golden altar before the throne; and the smoke of the incense rose with the prayers of the saints from the hand of the angel before God." - Revelation 8:3-4

It depends on the religion.

Some Christian denominations (such as Eastern Orthodox) use incense because Moses told the Israelis that they should use incense in worship. I assume it was supposed to make the Temple smell like Heaven, and many Churches still use it to remind people of Heaven.

Buddhists and Hindus seem to use incense too. i am not sure why, but it must be for a similar reason.

Incense, in general, helps the prayers get up to heaven with the smoke. It dates back to the OT. It probably had the practical effect of making the temple smell a bit less rank.
