would you address it Mr and Mrs. John Doe or Mr John Does and Mrs. Jane Doe? or just John and Jane Doe? and if someone has kept their maiden name Would it be Mr. John Doe and Mrs. Jane Smith?
oh, and i'll probably be putting Lord and Lady instead of Mr and Mrs (medieval theme)

For married couples with the same last name:
Mr & Mrs John Doe

For couples who are not married or have different last names:

Mr John Doe and Mrs Jane Smith

So are you still asking, or did you answer your own question at the end there?
What's proper for a married couple: Mr. and Mrs. John Doe. However, if I know the couple, I would use Mr. John and Mrs. Jane Doe.
If someone has kept their maiden name, she doesn't use "Mrs.", so she is Ms. Jane Smith.
For a shacked up couple: Mr. John Smith and Ms. Jane Doe.
For a single: Mr. John Doe and guest.

The correct way to address any invitation to a married couple is Mr. & Mrs. John Doe. That being said, if you are doing a theme wedding it doesn't really matter. How many of the people who you are inviting to your wedding do you think are going to say "Oh dearie me, can you believe it! She didn't address her invitations properly! The scandal!" I mean, common. Just make sure all of your invitations are mailed out in time, contain all important information, and tell your guest how you would like them to R.S.V.P. Good luck and congrats!
