For my twins (boy/girl) my family did a bear theme. It turned out really well.

How about....Peas In A Pod or Classic Pooh with Pooh and Tigger being the emphasized characters!! Or....Noah's ark "Two by Two".
Just make sure the invitations let people know it is twins and whether they are the same sex or not!! (I am sure most people will know already, though).
Treat the baby shower just like you would any other....the theme will carry the emphasis of TWO!! You don't have to carry the TWO all thoughout the shower....you don't have to have two punch bowls, two cakes, two bowls of chips, etc.
One item that might be really neat....is a custom diaper cake made just for the twins. I specialize in custom diaper cakes and I have made a few cakes for twins!! There could be enough items that could be doubled (booties, customized onesies, rattles, teethers, the plush topper) that the TWIN theme would definately be accentuated!! My identical twin boys are almost 5 months. Look up things that are just for twins. Two of everything doesn't always cut it. Two strollers or one twin stroller? Two highchairs or one twin highchair? They make a lot of stuff that is just for twins but you will definatly have more luck on-line than in a store. Also, lots and lots and lots of diapers. Designate one or two people to get premie diapers. I am sure they will be needed but not for long. People don't like to get just diapers, but that was our favorite gift. The people at my husband's work got together and just bought one package each. We haven't had to buy any yet. Maybe get some good cloth diapers. They make the best burp cloths because they are so absorbant. If you get bibs, decorative is nice but the kind with the plastic backing is great. They really save clothes from spit ups. Also, don't be afraid to move the party to the hospital if need be. I missed mine and had to send my husband alone because I couldn't leave the hospital.
Best of all. This will probably be the most appreciated gift. Offer to help with a few feedings and let mom get some sleep. The first few months are so so hard. Especially when the babies want to eat every two hours and not on the same hour. I didn't have very much help after the first month(3 weeks of which my boys were in the hospital). I really felt horrible for almost a month and was afraid of hurting them by dropping them or falling with them. I was tired and dizzy a lot from lack of sleep.

If you're throwing one, the same people will be attending as if it were a single shower. Just make a note on the invites to say it's twins (two boys, two girls, or a boy and a girl or sex undetermined) so people will bring two of whatever they're getting.
If you're buying for one, get something mom will really USE. Bouncie seats, I've yet to meet a person who didn't love the bouncie seat. LOTS of wipes and diapers or TWO diaper cakes and if you make them yourself then it's MUCH cheaper and you can throw in more personal items with them that you know the parents would like for their children. I didn't have twins but I had a baby boy 14 months after my daughter was born and the real nice things that I think I got were the simple things, the ones you go through a lot of. I could never have enough bottles (I've got 26 of them!) The BIG boxes of wipes from Sam's club, huge boxes of diapers for all sizes (include the gift reciept though because I got too many size ones and too many size 2's) boppy pillows for each baby with nice pillow cases so they can be washed constantly. Tylenol and orajel was always nice and those 1 second ear thermometers are great! No kid likes to wait around for their temp to be taken.

You need to buy 2 of whatever. Also what would be great is something to pamper the mother. This is a big step for her. For the babies something that is very thoughtful is medicines and things along that line.

One fun game might be things that come in pairs. Give everyone 2 minutes to come up with as many as they can. Some examples: socks, pair of pants, twins, lips, etc.
