how do i add my surname to my childs name?

i have checked depol and they only change and not add names. i wish to double barrel a name of a child

You CAN add your surname, provided both parties with parental responsibility of the child agree to it (or if you have sole parental responsibility). Effectively you aren't adding a name, you're changing to a new name which is double-barreled.

You need to get a "Change Of Name Deed" form. These are about 2.50p from a Legal Stationers (not a normal stationary shop-Look in the yellow pages). Just fill it in and get it witnessed by two people. Easy. Also, in effect you are changing your childs name because they are going to be known by another name to the one they have now.

I am assuming the child is already born and has been given a name you wish to change? In that case you have to go through the same steps as if you wanted to completely change their name.

If you hyphenate your name with the child's original surname this then becomes another name so the dipole should not have any argument about changing the name on the birth certificate

just change it notify all that need to know ie doc school child benefit etc and in two years its official.my kids medical records child benefit everything is in their new name.I didn't do this willy nilly by the way i sought advise from a solicitor
