If you've practice the best that you can. Believe in your talent that was given to you by God. So, ask Him to be with you during your performance and give you the strength to do your best and to keep your mind on what you are doing at all times. Then after it is over, thank Him for getting you through this performance.
Good luck.

Eat healthy, early nights and practice practice practice.
Eat things like fats & carbs before you go on stage help by giving you energy but dont eat too much as you can become bloated and feel sick!
Early nights help you concentrate more and more awake
Practice always makes perfect! The better you know the routine the better you'll be while performing!

Firstly, good luck!!
Remember, above all Practice makes perfect (just like everyone before me stated). The more confident you are, the better your performance will be.
Breath - deep breaths in and out will help calm you down and make it easier to focus on the performance.
And above all - SMILE. People will be so captivated by your smile, they won't notice if you do go wrong at any point!!

I totally agree with the first answer - my daughter has just finished performing at our local theatre and then went on to perform in her school's dance evening. Practice is all you can do - like I told my daughter if you make a mistake just carry on the audience will not know - good luck to you

Before you go on, if you are nervous, (I always was!), there is a pressure point that you can use to calm yourself down. Between your thumb and first finger, at the base of the "web"of skin between the two there is a hard "lump" of muscle, grasp this firmly between the thumb and first finger of your other hand and just close your eyes and breathe deeply. It does help, honest! Unfotunately, nerves in my experience are the main cause of less than perfect stage performances.
I won't say good luck...break a leg. (Metaphorically speaking)

PRACTICE, of course! Also, try performing in front of your mirror, and then trusted family and friends and ask for honest feedback...(and be prepared to take constructive critisicim...)...
Having the determination you already have does a lot to fuel your performance.
