There is no rating system for this. What is normally reported by observers is the size of standing army. On this basis neither US or UK make the top 5. During the Vietnam conflict, North Vietnam fielded the 3rd largest army in the world, behind China and India. The number for the Indian army is somewhat inflated because many of these troops serve as a national police, primarily to insure "domestic tranquility". China has a large force but its equipment is sparse and antiquated so its effectiveness on the world stage is questionable. I would say that the countries you would least want to war with would be US, UK and Russia, both because of the sophistication of weapons and air power, and their ability to mobilize additional resources during conflicts.

it really depends on what you mean by equipped for war.
are you asking what countries can project their power, i.e. fight a conflict halfway around the world.
if that is what you are asking my top 5 would be: the us, the uk, russia, then either france or germany.
if you are asking who can defend themself my top 5 would be: the us, the uk, china, russia, and either france, germany, or india.
if you take into account nuclear weapons i would say: us and russia are tied at first, the uk, france then china.
finally, if you mean just pure military power, if we were to take the entire military of a nation and put it on a piece of land with another nation's military on the other side of that territory who would win...i'd say: the us, the uk, china, russia, and either france or germany.
