The standard doxology is:
Praise God from Whom all blessings flow.
Praise Him all creatures here below.
Praise Him above, ye heavenly host.
Praise Father, Son, and Holy Ghost.

If you are looking for the music as well, it is contained in the UCC Hymnal. The lines above are the standard words. It is usually sung to the hymn tune "Old 100th".
